On August 9, 2023, the HQAI audit report identified a major corrective action request (major CAR M2023-8.4) during ACT Alliance's renewal audit. This led to the suspension of the CHS certificate, which was communicated to ACT Alliance on August 11, 2023. ACT Alliance then appealed the audit decision on August 24, 2023.

HQAI's Advisory and Complaint Board (ACB) reviewed all documents and records submitted by both parties. Following the review, the Appeal Panel decided to uphold the appeal. Consequently, the certificate was reinstated immediately, and the report was amended on November 20, 2023.
